Adult Recognitions

Adult Recognitions Form Used to nominate Girl Scout volunteers for Outstanding Leader and Outstanding Volunteer Award, Appreciation Pin Award, Honor Pin, Thanks Badge and Thanks Badge II Award

Letter of Endorsement for Awards Listed Above Used in conjunction with form above.

Youth Service Appreciation Certificate Nomination form for a group or roganization having offered a high level of support to Girl Scouting, satisfying award qualifications.

Leadership Development Pin Volunteers who have completed at least one year of leadership are eligible for this pin.
Camp and Program
Camp Staff Employment Application and References Used for applicants interested in working at either day or resident camp.
Camp Registration Used to register for day and resident camp.
Event Registration Used to register for any council event not including day or resident camp.
Facilities and Equipment

Facility Reservation Request Used to reserve the council properties.

Outdoor Equipment Reservation Used to reserve outdoor equiment.

Resource request Used to reserve program equipment.
Financial Grants

Financial Grant Request Used for troop/group dues, merchandise, and/or event requests of $30 or less.

Financial Grant Request Used for requests over $30. To be used with Recommendation for Financial Grant form.
Troop

Activity and Extended Trip Application Used for overnight activities, trips of more than 75 miles, for trips of three or more consecutive nights, camping on non-council owned property, high-risk activites, and sensitive issues.

"Bring a Friend" Special Event Permission Slip  

Activity Evaluation Form Used for trips more than 75 miles, overnight activities including camping, extended travel, and high-risk activites. Due at program center one week after trip.

Annual Troop Financial Report Each troop/group and service unit shall complete this form and turn it in to their membership specialist by June 15.

Community Service Troop Report To be completed after finishing each community service project.

Detailed Cash Record Used by troops as a cash record.

Membership

Insurance

All insurance purchased through the council covers all registered Girl Scouts and non-members. A signature from the council is required. Forms must be turned in two weeks prior to activity.

Plan 1 covers only registered Girl Scouts. This is paid for by the council and is good for any Girl Scout approved activity that lasts 2 nights or less (3 nights if one night is an official federal holiday). Only covers accidents, not sickness.

Plan 2 Plan 2 costs $.11 per person/per day. Good for any approved activity that lasts more than 2 nights (3 nights with federal holiday). Only covers accidents, not sickness.
Plan 3E Plan 3E costs $.29 per person/per day. Mostly used for resident camping, destinations and other approved trips lasting more than 2 nights (3 nights with federal holiday). Covers accidents AND sickness. Policy has a non-duplication provision.

 

Plan 3P Plan 3P costs $.67 per person/per day. Mostly used for resident camping, destinations and other approved trips lasting more than 2 nights (3 nights with federal holiday). Covers accidents AND sickness. Policy does not have a non-duplication provision.
Plan 3PI Plan 3PI costs $1.17 per person/per day. Used for approved international trips lasting more than 2 nights (3 nights with federal holiday)-strongly encouraged if less than 2 nights. Covers accidents AND sickness. Policy does not have a non-duplication provision.
